- At first, open a browser on your computer.
- Search the official website of Janma-Mrityu Tathya Portal.
- Then click on the Citizen Services, Death option, and select the Apply for New Registration option.
- Now, enter your Mobile Number and get verified with an OTP.
- Now fill in all the information correctly, such as death date, father’s and mother’s details, and full Address.
- Upload the required documents, such as the Aadhaar cards of the Father and Mother, the Hospital death certificate, and the Cremation or Burial certificate, and fill out Form 2.
- Now submit the form, and you will be given an acknowledgement number, which you can use to contact the hospital.
- Once the certificate is released from the hospital, you will be able to download it online.

How to Verify a Death Certificate
You can verify the death certificate of any person by following the steps below.
- At first, visit the official website of Janma-Mrityu Tathya Portal.
- Click the Citizen Service, then click the Death option and select the Death Certificate Verify.
- Then open a new page on your screen.
- Select the QR code verification or Certificate NO. Verification option.
- If you choose the QR code option, you can verify with your mobile camera. Or if you choose the Certificate NO verification option, you will need to provide the death certificate number.
- Now you will see the complete information on the death certificate. Here, you will also get the option to download the death certificate PDF.
Important Notice-
- After 21 days but within 1 year, apply with written permission from the local registrar and pay the late fee (pay the late fee online through Grips Portal).
- After 1 year, an order from an executive magistrate and an affidavit are mandatory.
