Who can do a Digital Birth Certificate?
- Certificates issued by the Panchayat or Municipality, or Government Hospital, after October 1999 can be digitised.
- However, the Birth Certificates issued by hospitals before 1987 may not be digitised.
How to digitize a Birth Certificate?
- You can not do this online.
- You will need to contact the relevant registrar’s office, like the Panchayat Office or Municipality Office, or the Government Hospital, with your original certificate and parents’ identity certificates (Voter Card, Aadhaar Card).
- Then the officials there will fill out “Form A” with your information and give you an acknowledgement number.
- Now that your Birth Certificate is digital, you can download your certificate online.

The following documents will be submitted for the application-
- Original old (handwritten) birth certificate.
- Certificate holder Aadhaar Card
- Father and Mother Voter Card, Aadhaar Card
Important Notice- If your original birth certificate is lost, but you have a photocopy of it or a polio card (where the registration number is mentioned), you can still get a digital certificate. In this case, you may have to submit a copy of the General Diary (GD) for a lost certificate.
What are the benefits of having a digital certificate?
- You can link your mobile number to your birth certificate.
- Once your certificate is digitised, you can easily download it as many times as you want using your registered mobile number.
- If there is any mistake in the old birth certificate, it can also be corrected by applying to the registrar along with the necessary documents.

- Visit the official website of Janma-Mrityu Tathya Portal.
- Click the Citizen Service, then click the Birth option and Track application option.
- Then enter the Application Acknowledgement Number and the child’s birth date and select the submit button.
- The current status of the application will then be visible to you.

- At first, visit the official website of Janma-Mrityu Tathya Portal.
- Click the Citizen Service, then click the Birth option and the Download Certificate option.
- Then enter the application Acknowledgement Number or the Birth certificate number and select the search option.
- Now, an OTP will come to the phone number provided in your application, which you will have to process using that OTP and download the Digitally Signed Birth Certificate PDF.

আপনি যদি আপনার বাচ্চার জন্ম সার্টিফিকেট ডিজিটাল করতে চান অর্থাৎ অনলাইনে চাপাতে চান তাহলে যে সংস্থা অর্থাৎ যেখান থেকে আপনার বাচ্চার জন্ম সার্টিফিকেট হয়েছে সেখানে যোগাযোগ করতে হবে। আপনার বাচ্চার জন্ম সার্টিফিকেট যদি কোন সরকারি হাসপাতাল বা কোন অফিস থেকে হয়ে থাকে তাহলে আপনাকে সেই হাসপাতাল বা অফিসে যোগাযোগ করে আবেদন করতে হবে।
নির্দিষ্ট অফিস বা হাসপাতালে যেয়ে আপনাকে হাতে লিখে আবেদন করতে হবে। সাথে নিয়ে জেতে হবে আপনার বাচ্চার জন্ম সার্টিফিকেট, আধার কার্ড এবং বাচ্চার বাবার ও মায়ের আধার কার্ড ও ভোটের কার্ড এর জেরক্স ও আসল কপি।
আবেদন করার পরে সেই অফিস বা হাসপাতাল থেকে আপনাকে একটি আবেদন নাম্বার দেওয়া হবে, যা দিয়ে আপনি পরবর্তী সময়ে অনলাইন থেকে জন্ম সার্টিফিকেট ডাউনলোড করতে পারবেন। তবে এই আবেদন প্রক্রিয়া আপনাকে অফলাইনের মাধ্যমে করতে হবে। এর জন্য কোন রকম অনলাইন প্রক্রিয়া শুরু হয়নি।
